• Insured’s name
• Insured’s date of birth
• Insured’s social security number
• Insured’s state of residency
• Policy number (if available)
• Date of disability
• Cause of disability
• Address to mail claim packet

What can I expect from the claims process?

Once your claim is submitted, it will be assigned to a claims representative for review. They will email or mail the correspondence to you and will request the documents necessary to review the claim for benefits.
